In the older infant room, there is not a sink in the diaper changing area.
There were cleaning supplies accessible to children in the cabinet below the sink in the inclusion classroom's bathroom. The hallway mop closet is accessible to children because it does not lock.
There was an infant sleeping in a crib with sheets that appear to be stained or soiled.
The unitary surfacing located on the playground was observed to be unkept.

In the school age room there was a cable outlet plate that was completely unscrewed from the wall exposing a whole and the dry wall. There were several paint chips in the after school room, 2 year old room, and the toddler room. Door frames and doors had visible dirt and grime on them. At the end of the building, closest to where the new construction is being done, the fence is missing. There are several boards along the fence line that are broken or pushed away from the fence. The changing mats in the infant room and toddler room were ripped leaving the foam padding exposed and unable to be sanitized properly. Note: Pictures were taken for the file.

There is a retention pond behind the parking lot and near where the school age children play. The pond is currently full of water due to recent rain.
