A child was left unattended in a restroom down the hallway from the classroom. The caregivers did not conduct a name to face count prior to leaving the restroom or upon arriving at their assigned classroom.
Based on the information provided, a child was wandering the hallway unsupervised.
A child was left in a classroom unsupervised; line of sight was lost when the door was closed, and class was in the restroom.
A caregiver did not double check to ensure she had all the children after leaving the bathroom and one child was left in the bathroom. Two additional caregivers walked into the bathroom; however, they were not responsible for this child and were not aware that the child was in the bathroom.

The Child Product Safety Commission form available was dated 09/06/2020. This is outside of the required annual update. This was corrected at inspection when the director updated and posted a new Child Product Safety Commission form which was dated 09/09/2022.
Garbage can in the three year old classroom was missing it lid. Lid was broken and garbage was accessible to children. This was corrected at inspection when director replaced garbage can with a working lid.
During of the course of the investigation it was proven a four year old child was left unattended in classroom for two minutes.

A child exited the playground gate without supervision or assistance from the caregiver after seeing parent at the operation.
