2 children younger than 18 months old were observed to be combined with children 2-3 years of age for a period exceeding 30 minutes.

A 2 year old child was observed leaving the classroom unnoticed. A caregiver was observed leaving a classroom unattended at least 3 times.

Infant observed crying for at least 10-15 minutes without a caregiver trying to pick him up and comfort him. The stopped crying when he fell asleep.

1/10 child files was missing release information.

1/10 child files were missing health information or signature.

1/10 files were missing their emergency authorization signature.

3/10 children have outdated shot records.

A child was observed alone in the hallway outside the classroom sitting in a high chair while the caregiver was in the room across the hall.

The Director and staff do not have current first aid cpr. training.
