There was not sufficient walking space between each childs cot during naptime in the two and three year old classrooms.

One caregiver was observed caring for 20 children ages three and four years old. The specified age group for the children is three. The classroom is out of ratio by 5. Also, one caregiver was observed caring for 13 two year old children. The classroom is out of ratio by 2.

The older infant and two year old classroom were observed out of ratio.

The director has not ensured that previous citations were corrected from the last inspection.

Three classrooms were observed out of ratio. The younger infant room SAG is 1 to 4 and was observed with five children. The older infant room SAG is 1 to 5 and was observed with 9 children. One caregiver was responsible for two groups of children.

An infant was observed with a bottle in the crib in the older infant room.

A bottle of bleacher was under the sink accessible to children in the infant classroom.

Bottles were not labeled in the infant room.

Caregiver with the 17-22 month old children had propped her phone on the table to play a video/music while the children sat at the table.

A caregiver was observed leaving a room and waalking down the hallway. The gate was left open and a child followed the caregiver into the hall.

One caregvier was observed in the room with children while their background check was still pending. This was corrected at inspection when the caregiver was sent home.

6/8 staff records reviewed were missing training for the following topics: Responding to allergic reactions, Emergency preparedness, handling hazardous materials.

Last fire inspection was in April 2020.

The standard was also evaluated and is part of the investigation. As a result of the DFPS investigation it was determine that a water dispenser having a hot water valve was placed in a location inside the operation that was accessible to children in care. As a result of the location a child in care received burns on her body from the hot water.

One caregiver was observed caring for 13 children ranging in age from 11 months to 23 months. The children were napping and the room was over ratio by 7. Another caregiver was placed in the room during the inspection.

This standard was reviewed as part of an investigation and found to be deficient. Based on interviews, the operation failed to notify the child? parents of the injury at pick up.
