I observe exposed wiring under a computer desk accessible to children in rooms #16 and #19.

I did not observe activity plans posted for each age group.

One caregiver present did not have personnel record available for review.

There was a change in site director that was not reported to child care regulations.

It has been found that a child was injury due to the caregiver not being able to keep the children under control.

You must use a Licensing Incident/Illness Report form, or other form containing at least the same information, to record information when a child needs medical attention after an injury. Incident form that was used is lacking Location of incident, licensing notification, medical attention information, doctor's information and recommendation, more details about the incident and privacy statement.

It has been found that children were not being supervised during the time of the incident. Caregiver did not intervene and did not see nor hear the incident while it was being taken place.
