Classroom caregiver was observed to step out of the classroom, leaving a group of 5 children unattended inside the classroom.

Four employee files were reviewed. Annual training for Director and one caregiver was not available for review.

Diaper changing supplies were within children's reach in three classrooms.

Four children's files were evaluated. Two files were did not have an immunization record. Two files were pending updated immunization records.

Feeding instructions for infants were not available for review.

Diaper changing mat in infant classroom is torn. It has a two inch opening.

Activity plans did not include outdoor activities. Activity plans for the current week were not posted in three out five classrooms.

Four employee files were reviewed. One employee was missing 10 hours of preservice training.

During inspection, one caregiver was observed supervising an infant group consisting of 5 children. The Specified Age Group was 0-11 months. Two caregivers are required to supervise the group. The second caregiver had stepped out of the classroom, leaving the child/caregiver ratio deficient. Note: It was corrected during inspection when the second caregiver returned to the classroom.

Two infants ages 7 months and 10 months were observed sleeping in bouncers. Note: Infants were moved to cribs during inspection.

Three individuals were observed supervising children in the infant, K2, and K4 classrooms did not have active, eligible background checks on file. Individual in the infant classroom was counted in child/caregiver ratio. Note: It was corrected during inspection. Individuals left the operation during inspection.

Trash can containing diapers was uncovered in infants' room. Trash cans in restroom used by children were uncovered.

There was a slide in the outside playground, with a platform of 22 inches with no surfacing. Note: It was corrected during inspection by moving the slide to the area of the playground that has unitary surfacing.

Four employee files were evaluated. Two records were pending the required employment affidavit.

Four employee files were evaluated. Two employees completed their First Aid/CPR training 100 % online. The training did not included practice on manikins.

Electrical outlets in two clasrroms were observed to be uncovered.

The operation does not have the following on their operational policies: -Procedures for parental notifications -Suspension and expulsion of children -Safe sleep policy for infants from birth through 12 months old -Hearing and vision screening requirements -Enrollment procedures, including how and when parents will be notified of policy changes -Water activities -Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter including (A) - (F) -How the parent may access the minimum standards online -Instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website -Your emergency preparedness plan -Your provisions to provide a comfortable place with an adult sized seat in your center or within a classroom that enables a mother to breastfeed her child. -Preventing and responding to abuse and neglect of children, including (A) - (E) -Information on vaccine-preventable diseases for employees -Inform the parents that they may visit the child-care center at any time during your hours of operation to observe their child, the child-care center's program activities, the building, the premises, and the equipment without having to secure prior approval, and -Under the Texas Penal Code any area within 1,000 feet of a child -care center is a gang-free zone, where criminal offenses related to organized criminal activity are subject to a harsher penalty.

Outdoor activity space is not ready for use by children. The space has clutter and there is not a clear path to access the outdoor space without having to use the parking lot area.
