Six children's files were evaluated. Three files did not have the immunnization records.

There is a diaper changing table in the pre-k classroom. The classroom does not have a sink available for hand washing.

There were two buckets containg water outside of the children's restroom. Buckets were accessible to children. Three outlets in the pre-k classroom were uncovered. Note: Buckets were removed during inspection. Outlets were covered during inspection.

There was no additonal employee in the center aside from the ones required to supervise the classrooms during naptime.

Four employee records were evaluated. One employee completed the CPR training 100 % online. The training did not include the use of a CPR manikin and hands-on skill-based instruction, practice, and testing.

Two infants were observed sleeping on restrictive devices. Caregivers did not move children from devices to their cribs as soon as possible. NOTE: This was corrected as caregivers moved children to their designated cribs upon being informed by CCR Inspector.

A toddler child was observed to be falling asleep inside a high chair device attached to a small chair during nap time. Child was leaning towards the front of the chair. Caregiver providing supervision did not move child until being informed. NOTE: Caregiver moved child and placed him in a sleeping cot during inspection.

A caregiver was inside the toddler classroom assisting another caregiver on 8/10/2021. An eligibility letter for this caregiver was not received until 8/11/2021. NOTE: Eligibility letter for caregiver was received on 8/11/2021.

Immunization records were not available for review for 2 out of 6 children's records.

There were 12 children of specified age group 18 months - 23 months old under the care of one caregiver. NOTE: This was corrected during inspection as director stepped into the classroom to meet child/caregiver ratio.

3 bottles were not labeled with children's names in the infant classroom as observed during inspection. NOTE: This was corrected during inspection as bottles were labeled by caregiver.
