Director was missing 21 hours of the required annual trarining.

Caregiver did not know the number of children she was supervising. Children were observed sleeping in two different classrooms. One caregiver was supervising both groups of children.

Trash cans in the restrooms used by children did not have lids. Garbage was accessible to children. There were chemicals on top of the sink in the restroom used by children.

Carbon monoxide and smoke alarm detector was last tested on February 2022.

The sanitation inspection was last conducted on 04/29/2021.

Parents were not notified in writing there would be a dog at the operation. A dog was present at the operation during inspection. The dog was inside a crate.

Fire extinguishers' monthly inspection was last condcuted on February 2022.

Fire inspection was last conducted on 05/03/2022.

Monthly fire drills were last conducted on Februrary 2022.

Mats used for naptime were observed to be numbered. Number/child assignment map was not available for review. Note: It was corrected at inspection.

Children's products certification form was not available for review.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

The following training topics were not available for review for one caregiver: -Abuse and Neglect. -Medication Administration. -Preventing and responding to emergencies due to food or an allergic reaction. -Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic. -Handling, storing, and disposing of hazardous materials.

Director was missing 20 hours of training in the most recent complete annual training year.
