Activity plans in four classrooms only included one opportunity for outdoor play. Activitiy plans in three classrooms did not include the dates and group names. Activity plan was not available for the school age classroom.

Child care operation sign in/sign out log was not maintainted, The last day it was completed was on 11/28/2022, and the sign out time was not documented.

Operational policies were pending information on a plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play, local abuse and neglect organizations, hearing and vision requirements, updated immunization records, and field trips.

The operation has a step-2 Clubhouse Climber in the outside playground. The designated play surface has a height of 23 inches. The operation has turf grass, under the equipment.

Employee observed supervising children did not have an active, eligible background check on file. Note: employee left the center during inspection.

Six children's files were evaluated. One file was missing the required admission information.

Infant group of children was observed watching television. Note: it was corrected during inspection, by turning off the television.

Required monthly test for carbon monoxide detector, and monthly inpection of fire extinguishers have not been conducted since January 2022.

List of the children being transported and emergency medical transport and treatment authorization forms for each child being transported were not included in information carried in transportation vehicle. First aid kit was missing a thermometer. Note: Information was included in transportation binder during inspection.

Small school bus used for transportation is not equipped with the required electronic child safety alarm.

Health environmental inspection was not available for review.

Garbage cans in classrooms were uncovered. They did not have a lid. Garbage was accessible to children. Cleaning supplies were inside a closet. It was unlocked. Closet is in school age classroom.

Activity plans posted in one toddler classroom, and one pre-k classroom were for previous weeks. Activity plan in school age classroom was not available for review.

Carbon monoxide monthly test documentation was not available for review. Fire extinguishers' monthly inspections documentations was not available for review.
