During opening, the center had two classes. In one class one caregiver was observed supervising 15 children. Four children were infants, ranging from ages 12 months to 17 months. The maximum number a caregiver with 4 infants of that age group is 5 children. Opening group was divided two hours after opening the center. In the other class one caregiver was observed supervising 7 children ranging from ages 3 months to 15 months. Specified age group was 12-17. The maximum number one caregiver can have, based on the children's ages, is 5 children.

Eight employee files were reviewed. One employee hired 3 months ago did not have the pre-service training. Two employees were pending training in the following topics: administering medication;preventing and controlling the spread of communicable diseases, including immunizations; understanding building and physical premises safety; and handling, storing, and disposing of hazardous materials .
The operation had a climb and crawl catepillar on concrete. It did not have the required surfacing. Step 2 structure with a slide of 27 inches in height did not have the required unitary surfacing.
Infant classroom was out of child/caregiver ratio. There was one caregiver supervising 7 children ranging from ages 12 months to 24 months. Because the specified age group was 12-17 months, the maximum number of children one caregiver may supervise is 5 children. Note: it was corrected during inspection.
Eight children files were reviewed. Six children were pending updated immunization records.

Based on evidence gathered during investigation, one caregiver was taking a virtual class on a computer while providing care of 2 children during hours of operation.
