There was a fan in the pre-k classroom, on top of an approximately 5 foot shelf. Fan was not mounted. Note: it was removed during inspection.
There were 5 children under the care of 1 caregiver. The youngest child was 7 months old, the oldest child was 2 years and 4 months old. The 18 month difference was exceeded. There were 13 children in care at the operation. NOTE: This was corrected during inspection as the two older children in the room were moved to be under the care of another caregiver.
One probation condition was deficient during inspection: Building and Grounds check documentation was not available for review for 11/1/2021.
Both staff members in charge of a group of children during nap time were observed engaged in other activities not related to the care/supervision of the children. Staff members were using their laptop computers. NOTE: This was corrected during inspection. Staff members put away their laptop computers.
One napping mat in use during inspection was not labeled with child's name/number. NOTE: This was corrected during inspection as mat was labeled by Director of operation.
The door frame and door knob of two classroom doors, one restroom door, and the refrigerator door handle were observed contaminated with matter and dirt was visible. NOTE: This was corrected during inspection. All areas were cleaned and disinfected by Director of operation.
One out of four staff records reviewed were missing the following trainings for a caregiver: Administering Medication Allergic Reactions Emergency Preparedness Handling, storing, and disposing of hazardous materials.
Building and Grounds checklist was not completed today, 5/13/2021, before 9:00am as established in the condition of corrective action plan.
Infant daily report was not being completed for one out of three infants enrolled at operation and present during inspection.

Three caregivers and director did not have CPR training that included the use of a CPR manikin and hands-on skill-based instruction with practice.
