Fire inspection was last conducted on 12/22/2020.

Fire drills were last conducted on April 2022.

Four infants were in a combined group with 10 additional children over the age 18 months, under the supervision of one caregiver, during opening of the center.

Monthly carbon monoxide tests and monthly fire extinguishers inspections have not been conducted.

Fire extinguishers wer last serviced on December 2020.

A person authorized to pick up a child, by the child's parent, was not allowed to enter the operation.

Two infants sat on high chairs for longer than 15 minutes, during opening, while waiting for infant caregiver to arrive to the operation.

One toddler was observed to be sitting down on a high chair for longer than 20 minutes as a form of discipline.

Ten children records were reviewed. Three records were missing updated immunization records. Note: it was corrected at inspection. Updated immunization records were provided by parents to operation during inspection.

Activity plans were not posted in three classrooms. Note: it was corrected during inspection.

There are two sinks in the infants' room. One is designated for hand washing and one for rinsing bottles. Sink for rinsing bottles was observed being used for hand washing. Note: It was corrected at inspection.

After diaper change, caregiver and child were observed not washing their hands. Note: It was corrected at inspection.

Infant classroom was out of child/caregiver ratio. There were six children under supervision of one caregiver. The specified age of the children in the group is 12-17 months. Note: it was corrected at inspection.

Two desinfecting chemicals were inside an unlocked cabinet under the sink in the infants room.

Emergency practice drills have not been conducted.

Two caregivers were observed caring for a group of 12 children with a specified age group of 0-11 months.

Active play space and equipment monthly inspections have not been conducted since January 2021.

Six employee records were reviewed. Two caregivers completed their First aid/CPR training 100 % online. A new caregiver, without First aid/CPR certificate on file, was observed supervising the school age classroom.
