There was a disinfecting spray, under the sink in the studio's restroom. Cabinet was unlocked. In the toddler room, underneath the diaper changing station there was an unlocked cabinet with cleaning supplies accessible to children. Inside the studio, there was an electrical fan dryer on top of a cabinet accessible to children. Two water hoses in outside playground were accessible children. NOTE: This was corrected at inspection.

It was observed inside the studio's restroom that the trash can didn't have a lid, making its contents accessible to children. Playground equipment was in need of cleaning. NOTE:Trash can was removed from the restroom. Playground equipment was cleaned during inspection.
The Consumer Product Safety was not made available to licensing at the time of inspection.
In the infant room, cribs were observed to be right next to each other making infants able to reach into each other's crib. NOTE: This was corrected at inspection.
A child was observed to go to the restroom unsupervised. The restroom is located outside of the classroom.
Eight out of 10 employee records were observed to be missing the CPR training. The CPR trainings provided was done 100% online. Mannequins were not used on the trainings.
Three cribs were observed to have oversized mattress sheets. Sheets were observed to be tucked under the mattress.
Operation notified parents approximately 72 hours after becoming aware that a child in their care had contracted a communicable disease deemed notifiable by the Texas Department of State Health Services. Operation is required to notify parents within 48 hours.
