Based on video footage from 12/28/2023 caregiver pushed child?s head down on table as a form of corporal punishment.

On 12/28/2023 classroom was observed via video footage to have one caregiver to ten children between the ages of 12 to 18 months.

Staff first name and or initials only were observed on medication log for staff who administered medication.

Based on monitoring inspection observation director has not ensured all employees are mantaining compliance with minimum standards.

During inspection, two children did not have current immunizations on file during inspection.

Administration of unassigned epinpherine auto-injectors was not listed in Operational Policies.

During inspection, one child was missing emergency address, and another child was missing emergency contact phone number.

During inspection, one child was missing authorization of medical emergency information.

During inspection, one child was missing date of admission.

Promotion of Physical activity was not listed or discussed in Operational Policies.

Last Fire Inspection was condcuted on 4/18/2022.

An employee hired on 8/15/22 does not have current background eligibility results on file. This employee was working with children in a classroom. An employee hired on 9/1/22 is working in the operation without background check eligibility results on file.

An incident occurred which resulted in two children's medications being swapped. The two children have the same first name.

The operation gave a two-year-old child Children's Benadryl and Hyland' s Kids Cold & Cough at the same time as authorized by parents. The label instructions for Children's Benadryl medication states "Children ages 2-5 years old, do not use unless directed by a doctor."

During visit, below are staff that have not completed annual training for 2021-2022 year. Staff hired on 8/25/2021 had 12 hours. Staff hired on 10/21/2020 had 6 hours. Staff hired on 8/10/2021 had 11 hours. Staff hired on 8/25/2021 had 7 hours.

The caregivers in the two-year-old classroom were not actively involved in supervising their group of children and a child was injured. One caregiver was observed using an iPad to take pictures of the children?s activity and the other caregiver was preparing the next activity taking the caregiver's attention away from the children. One child pushed another child causing an injury.

A caregiver did not demonstrate good judgement when moving a child from a chair to the floor during lunchtime.
