During follow up inspection, staff hired on 6/25/20 had completed 19 hours of annual training during the 2020-2021 year. Staff is currently missing 5 hours.

Operation had expired epinephrine auto-injector. Injector expired 4/13/2020.

During inspection, one child was missing emergency medical authorization information.

Director did not have photo identification on file.

Staff hired on 7/1/19, 3/1/11, 4/15/19, 4/26/16, 2/1/21, 6/25/20, 3/21/22 and 9/18/20 did not have high school diploma or G.E.D on file

During inspection, one child did not have an updated feeding instructions for the month of May.
