Corrected: Nov 3, 2023
This standard was reviewed on 10/3/2023 as a follow-up to a previous inspection and found deficient. Through out the school age playground patches of the black tarp are pulled and have become tripping hazards. In addition, large cracks in the ground were observed on the school age playground that present as a tripping hazard.
While walking the playground it was noticed that the bottom of the fence that borders the busy street is pulling up. Little orange pegs were put to keep it down but they are not holding. It was noticed there is a big gap in the gate. The gap is big enough that a preschooler could fit through. Through out the school age and preschool playground patches of the black tarp are pulled and have become tripping hazards.
While walking the school age room I observed video games with violence on the game shelf.
While doing my walk through at lunch time I observed the 2's teacher call the kids over to the lunch table directly from an activity and not have them wash their hands.
This standard was reviewed as part of an investigation and found deficient. It was determined that children were allowed to play on play equipment while on a field trip that allowed children to fall inside the structure and onto other parts of the structure. This resulted in a child being injured and obtaining medical attention.
This standard was reviewed as part of an investigation and found deficient. It was observed on video that a caregiver inappropriately disciplined a child in care.
This standard was reviewed as part of an investigation and found deficient. The operations Incident Report was reviewed and did not contain the same information as the Child Care Regulation's Incident Report Form (7239).
The gates to the a/c units on each of the playgrounds did not have anything to prevent children from opening the gate and having access to the a/c units. Two gate that open up to the school age playground that leads to the parking lot did not have anything to prevent children from opening the gate and escaping to the parking lot. Children have access to two laundry rooms that contain cleaning products. In the Pre-K class and in the school age building there is not a lock or child proof cover on the door to prevent children's access to the water heater. Grass was observed growing in the loose fill on the toddler playground which makes the loose fill surfacing non resilient if a child were to fall.
This standard was reviewed as part of an investigation and found deficient. The operation failed to self-report to Child Care Regulation within 48 hours of becoming aware that a child obtained medical attention as a result of an incident where the child was injured while in care.
The fence around the play ground has gaps that prevent it from being secure. There are parts of the fence that have damage resulting in sharp points that are a hazard and offer a great risk of injury.
Play equipment on the younger playground has coating that is broken and chipping that can cut children's fingers. A piece of the train play equpment is broken and completely detached from the play equipment and needs to be removed from the playground.
Operational policies/parent handbook needs to be updated to include the benefits, the duration, the type of physical activity, the setting the physical activity will take place and a plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.
