One caregiver lacked training in understanding early childhood brain development.

One caregiver stated that infants are not engaged in outdoor play for a duration that each infant can tolerate.

One child aged 4 years did not have a hearing and vision on file.

Upon reviewing 10 children s records, two children did not have updated immunizations records available.

The playground structure was observed to have spider webs on it.

Five out of ten caregivers were found to lack training in emergency preparedness. Six out of then were lacking training in Preventing and controlling the spread of communicable diseases, including immunizations; Preventing and responding to emergencies due to food or an allergic reaction; and Understanding building and physical premises safety.

At the time of inspection, three infant in care did not have a written feeding instruction on file or available for review; the infants were not ready for table food.

1 out of 10 child files reviewed did not contain current health statements from the child's physician after being enrolled for 12 months.

Four medications in the medication box were found to be expired, including one EpiPen that had passed its expiration date. Pitures were taken.

The director lacked training in Emergency preparedness, Administering medication, Handling, storing, and disposing of hazardous materials.

Loose fill was less than 3 inches for slides on the playground which require at least 6 inches of loose fill. This deficiency was corrected at inspection when the director had the maintenance crew rake required amount of loose fill around the use zone areas.

The building was observed to have large cracks in the walls, floor, and ceiling.

Playground equipment has not been adequately maintained, creating a risk for children when utilized. Loose boards and gaps were observed on a play structure used by young children.

At the time of inspection the operation did not have a current fire inspection.

Exposed Nails were observed in an area where young play.

Three children did not have a food allergy plan in the file at the time of inspection.
