The operation does not have a current fire inspection. It was last completed February 2022.

Only the director had education qualifications on file. None of the five caregivers had education qualifications on file.

Four caregivers who have been working for more than 12 months did not have 15 hours of annual training.

Three caregivers did not have First Aid and CPR certification on file. Two caregivers had expired First Aid and CPR certification.

The operation did not have a current health inspection. The last health inspection was completed December 2021.

A review of liability insurance data from March 3, 2023, revealed that the operation had not updated their liability insurance as required.

Three of seven staff did not have an employment affidavit on file at the operation.

Observed mutiple cleaning supplies in the reach of the children to include Odoban, Ant spray, and lysol. There was also multiple cans of spray paint and cans of paint on the floor.

The enrollment forms do not have permission from the parents of the children for transporation from school to the operation.

Observed the fire extinguishers not being serviced since September 2020.

There was no fire extinguisher in the vehicle used for transportation.

Observed a room where children were walking that had toys and material on the floor, which causes a tripping hazard.

There was medication in the reach of the children in care. The medication was removed from the area.
