During a DFPS investigation there was sufficient evidence to support that an infant was able to climb out of a highchair and hit their head on the floor. One caregiver walked away from the infant sitting in the highchair, had their back towards the infant, and was using their personal cell phone. The other caregiver was facing the infant while sitting on the carpet and failed to get up and stand in front of the highchair knowing the infant has attempted to get out of the highchair previously.

Information obtained during a DFPS investigation shows that a caregiver failed to properly fasten the safety straps on a highchair as the safety straps were fastened, but not tightened resulting in an infant climbing out of the highchair and falling to the floor, hitting their head.
