Three children in care stated they were spanked by foster parents. The foster parents admitting to spanking one foster child on the hand using their hand and spanking the other two children on their hands/bottoms using their hand or a rubber spatula.

During the sampling inspection, the playground equipment was observed to be not well maintained and needing repair. There was missing wood and protruding nails.
During the sampling inspection, it was discovered the neighbors babysit the child in care but has not completed a background check with the agency.
One of the four child records had a Preliminary Service Plan that was thirteen days late.
Both child discharge summaries did not include the current medication at the time of discharge. The summary indicated the medication regimen as n/a, however, the medical forms for both children show they are currently taking a prescribed medication.
Two of the four records reviewed indicated the two children were prescribed a daily medication, however, medication logs indicated "none at this time." There is no evidence on medication logs supporting that medication was administered daily. One child record indicated a child was prescribed a daily supplement, however, there is only one record indicating that the child was administered the medication. The one child record does not show that this supplement was discontinued.
One of the four child records reviewed did not have child/parent signatures indicating child rights had been reviewed within seven days of the child's admission.
