It was found during the investigation that the Director of the operation was not ensuring the operation maintain compliance with Minimum Standards by allowing classrooms to be out of ratio, not supervising employees appropriately, not ensuring the employees are maintaining compliance with Minimum Standards, and providing falsified documents to Licensing.
