It was observed in a classroom that the diaper changing mat material was cracked and missing in areas and the foam in the mat was exposed.

1 out of 10 records reviewed did not have a current immunization record in their file.

2 out of 10 records reviewed was missing an immunization record in their file.

It was found during the investigation that a child was left unsupervised on the playground for approximately 5 minutes.

It was found during the inspection that a slide on the south playground was cracked and posed a pinching hazard. This was corrected at the inspection by putting tape over the cracks until they can replace the slide.

It was found during the investigation that a child in care had access to and swallowed an object that is considered a choking hazard.

Based on the evidence obtained a child in care was pinched by a teacher, causing bruises.

3 out of 10 records reviewed was missing immunization records or updated immunization record.

1 out of records reviewed did not have current First Aid Training.

1 out of 10 records reviewed did not have current CPR Training.

1 out of 10 records reviewed did not have a Health Statement and has been attending the daycare for over a year.

It was found during the inspection a 5 month old infant was swaddled in a crib. This was corrected at inspection.

It was found during the investigation that a caregiver was inappropriately discipling children.
