There was a classroom of children ages 20 months to 4 years old and they were watching a television show that was on the lesson plan. There was one child under the age of 2. Television is not permitted for children under the age of two. It was corrected at the time of the inspection by the caregiver turing the television off.

This standard was evaluated regarding a report received and found to be deficient. Evidence received through interviews supports the fact that a caregiver did not properly supervise a child resulting in the child being left on the playground alone for 35 minutes.

There were three trash cans with no lids in the operation that had trash in them that consisted of diapers, drinking cups that had been used and hand washing paper towels. The Director corrected this by getting new trash cans with lids.

It was found during the DFPS investigation inspection that two infants were left two sleep in restrictive devices.
The child that was swaddled did not have the proper documentation-the Sleep Exception Form that allowed the child to be swaddled when sleeping.
Both caregivers in the infant room did not have the proper documentation stating they received safe sleep training.
During the DFPS investigation inspection, speaking with staff and reviewing video footage three children under the age of 12 months were seen sleeping in cribs with blankets.
There was not a fire extinguisher in the van. The Director corrected this at the time of the inspection by putting a fire extinguisher in the van.
In the 14-19 mo old room there were 8 children with one caregiver. The ratios are 1 teacher to 5 children for that age group. This was corrected during the inspection by the director having another teacher go into the room.
Based on interviews and video footage, it was found that all of the children ages 3mo-7 yrs or older were being put in the infant room with one teacher and were out of ratio and it was also seen when a 2nd teacher came into the room, the room was still out of ratio.
Based on the information obtained through interviews and viewing video footage, the operation was combining children that had an 18 month age difference. It was see that children 3mo were combined with 7 year olds for at least 2 hours in the mornings.
The Director was involved in a minor car accident with a 4 year old child in the vehicle. The Director called Lufkin Police Department, but the operation did not notify Licensing.
A caregiver was seen on video twice walking out of the classroom for approx. 20 seconds leaving children 2-3 year unattended.
A caregiver was seen on video pulling a child up from off the floor by his arm in a harsh manner.
