Based on the review of video footage and staff interviews, it was determined that a lapse in supervision occurred when a vape device fell from a parent's pocket onto the floor of the infant room. During this time, a child was able to pick up the vape and put it in their mouth.

There was a water hose on the playground that was not put up. It was laying by the fence covering the airconditioner. This could be a tripping hazard. Photos were taken. The Director corrected this at the time of the inspection by putting the waterhose up.
One caregiver that has been transporting transportation training expired on 3-19-2021. The Director is going to correct this by having the caregiver complete the training before she transports any children and submit the training documentation to Licensing.
A caregiver had a school age child do wall sits while holding a book in their hand as a form of discipline. This type of discipline is a prohibited and humiliating type of discipline. The Director has a plan on trainings with staff on proper types and positive types of discipline that she is going to submit to Licensing.
