The caregiver did not use good judgement when the child fell off of the slide. The caregiver previously told the children not to run down the slide, however the caregiver was not in position to intervene when the child began to run down the slide.

Based on the information obtained during this investigation, the operation failed to conduct daily maintenance procedures, resulting in a child getting out of the fence unattended. Note: The fence at the operation associated with the previous citation issued was repaired. However, during the inspection on a different playground, a large opening was observed on the fence. Also observed, was a metal fence and pole leaning over the playground area.
