There was an 18-month difference in age between the youngest child, 4mos, and the oldest child, 24mos, when the director arrived at the operation and moved the toddler into the infant room.

Signed sleep exception form was not available to review for the infant who was observed sleeping on his stomach in the crib.

Observed 6 infants, ages 4 to 11 months old with one in the infant classroom and one caregiver supervising 11 children, ages 17mos to 23mos in the toddler area/room.

The six certificates provided by the operation were falsified. American Heart Association and Austin Travis County EMS verified that the AED and CPR certifications provided by the operation are not valid for the 6 staff members.

Observed a blanket in the crib with a 5-month-old.

Feeding instructions for the infants not on table food was not available for review.

During the course of the inspection, the inspector found that the director?s training certificates, first aid and cpr training for six staff members and the annual fire inspection were falsified.

Travis County fire verified that the annual fire inspection was not completed in 2023 and the inspection form provided from 2022 was altered to show a 2023 inspection date.

Observed backpacks of children belonging on the floor near the door in the infant room.
