Based on information obtained during the investigation, it was determined that operation staff waited until pick up time to notify the parent that their child had been injured. That injury required medical treatment.

The garbage was picked up prior to children going outside in the afternoon. 4. Two classrooms had cleaning supplies on a cart and on the classroom sink accessible to children.
