Based on information obtained during the investigation, it was determined that operation staff waited until pick up time to notify the parent that their child had been injured. That injury required medical treatment.

The floor in the restroom is in need of repair and not been fixed at the time of the follow up inspection.

The operation did not provide a record of the last pest extermination during the follow up inspection.

The operation did not document when the carbon monoxide was tested since January 2021. A carbon monoxide in two classroom was beeping during the follow up inspection.

Out of ten records reviewed, three children were not current on their immunizations.

Out of ten caregiver files reviewed five caregivers were missing the annual required training at the time of inspection.

1. The toilet in one classroom was not working, and needed to be cleaned. A outdoor carpet outside was coming up on the end which may cause a person to trip. 2. The floor in the restroom is in need of repair. 3. Paper cups and garbage was left on the playground that was used earlier in the day. The garbage was picked up prior to children going outside in the afternoon. 4. Two classrooms had cleaning supplies on a cart and on the classroom sink accessible to children.

The operation did not provide a record of the last pest extermination during the inspection.

A fire extinguisher in a classroom was not mounted on the wall.

The director's training was not available for review at the time of the inspection.

The operation did not return or dispose of medication that was expired.

The operation did not have documentation available for review during the inspection that verified the carbon monoxide detectors had been checked monthly. The last documented date available during the inspection was January 2021. A carbon monoxide in one classroom was beeping during the inspection.

Loose fill on all three playgrounds have not been maintained. Loose fill needs to be added to maintain the 6-9 inches requirement.

The caregiver had a pending background and received notification fingerprints were needed. The caregiver was at the operation prior to receiving a background check status result of eligible, eligible with conditions, or provisionally eligible. An administrative penalty in the amount of 100% of the daily maximum per HRC 42.078(a-2) will be recommended as a result of this citation. An additional penalty in the amount of the daily maximum will be recommended for each day past the compliance date that the deficiency is not corrected. It is your responsibility to notify licensing when you have corrected the deficiency.
