The operation was aware of a child inappropriately touching other children in the past, and due to lack of supervision it resulted in another incident of inappropriately touching.

Based on information obtained during a DFPS investigation, it was determined that caregivers were unaware of injuries a child sustained while in care.
There was no emergency evacuation and relocation diagram posted in the toddler classroom. (Note: The diagram was posted during the inspection.)
The operation did not have a current certification form showing recalls have been reviewed. (Note: The director corrected this during the inspection when the form was completed & signed. The form is now available for review.)
The operation did not have a current fire inspection on file. The last fire inspection was completed 2/2021.
The following hazards were observed: 1. There was a bottle of disinfectant spray sitting on the toilet in the toddler classroom accessible to children. (Note: The caregiver moved the chemical and put it in a cabinet that was inaccessible to the children.) 2. There were pieces of a broken toilet tank cover on the floor in one of the preschool restrooms. (Note: This was removed during the inspection.)
It was determined that on more than one occasion, infant children have remained in a restrictive device for more than the 15 minutes allowed by the Standards.
