Based on information obtained during an investigation, it was determined that the childcare facility did not have an Incident Report regarding a situation that placed a child at risk when a child was placed in a chair in a harsh manner.
There was sufficient evidence found during a self-reported investigation to show that a caregiver used prohibited punishment when they placed a child in a chair in a harsh manner.
A child was found unsupervised in an empty classroom for approximately 40 minutes. This was corrected at the time of the inspection when all caregivers were observed supervising in compliance with minimum standards. The caregivers were counseled and retrained on safety and supervision. The caregivers were retrained on the required transition sheets.
One employee was present without an active background check. The director stated the employee was accidentally taken off the operation's active list. This was corrected at inspection when the director had the employee leave.
Based on the information obtained during an investigation, it was determined that the operation did not immediately notify all parents when they learned children may have been placed at risk when a child had a weapon on the daycare van and at the daycare. A notification was sent out to parents two days later and only to parents of school age children.
