During the investigation it was found that a piece of play equipment was broken and cracked and was on the playground at the time of the incident. The equipment was pushed up into a corner out of required use zone near a fence.

During inspection a bottle was observed in the crib with a sleeping infant and four bottles were observed on the floor throughout the room.
During inspection a blanket was observed coving the changing mat and diapers were changed without removing the blanket and santizing for the next child.
During inspection an infant was observed asleep in a play yard that had a side that was not locked into place.

During the investigation four caregivers were observed in the room playing with the babies and two of the infants were sleeping in their bouncers and not moved to their play yards.

During investigation 8 infants did not have updated feeding instructions. Some children were provided extra food from the classroom instead of the food parents provided.
During investigation it was found a caregiver uses electronic devices for personal business while caring for children.
* The infant room has baseboard area that is peeling and needs to be repaired. * The Prek room has an electrical outlet box that has pulled away from the wall and needs to be repaired. * Two year old room has outlets that are not covered.
During the investigation it was found that the operation used a cream on a child that was not approved by the parent.
During inspection a current health inspection was not available.
During inspection a classroom was found out of ratio. Children were combined in other classses to put this class into ratio during inspection.
A current fire inspection was not available during inspection.
During inspection the playground had the following safety issues: Large slide piece has large crack on the platform - remove or repair before allowing children on it. Plastic slide requires loose fill but was removed from the playground during inspection Pieces are not 6 feet apart from fence or from each other. A sandbox cover has bolts in four corners that need to be removed or covered.
During the investigation, pre-service training documentation was not available for a caregiver.
This Minimum Standard was reviewed and found to be out of compliance. An individual who had a pending background check was observed at the operation inside of a classroom.
During inspection three children were observed napping in an area that was unsupervised and one child was found napping alone in a room unsupervised.
During inspection a caregiver did not wash her hands or the child's hands after changing a diaper.
During inspection children were observed to be sitting in high chairs for over an hour without an activity or being able to get down and be active.
During the inspection a child was observed leaving the area with the children and caregivers and going down the hall several times towards the door to go outside and in and out of a classroom several times without caregivers being aware that the child was out of the room.
During inspection a classroom has cleaning chemicals within reach of children and a closet is accessible to children with chemicals stored inside.
Duiring inspection a carbon monoxide detector was not installed in the building.
During inspection it was found that the playground areas did not have a fence installed.
During inspection personall storage for individual belongings was not available for children.
