During the investigation it was found that a parent reported that a caregiver was pinching a child but the operation did not report to licensing.

During the investigation it was found that there are many ceiling tiles in the each room that are stained from the roof leaks and some have mold. There are walls that are water damaged and need repair.

Based on a DFPS investigation, it was determined that caregivers grabbed a child on at least 2 occasions as a form of discipline.

During the investigation it was determined that the caregiver popped a child on the hand as a form of discipline.

During inspection it was observed that children did not have outdoor playtime.

During inspection an infant was observed to be in a bouncer for 40 minutes before being removed.

During inspection, children did not wash hands after playing outside and there is no soap in the restrooms for handwashing.

During inspection, the following items were not found in the chidlren's records: 746.605(6) Name, address, phone number of emergency contact for all 4 children Child #1 needs the special care statement box checked Child #1, #2, and #4 need the parent handbook (operational policies) boxes checked. Child #4 needs the healthcare Provider Statement or parent check box completed.

During inspection, four infants did not have required feeding instructions.

During inspection, children under two were able to watch a video on a tablet.

During inspection, three pack n plays were observed with very loose fitting sheets. These sheets were removed during inspection.

During the inspection an infant was observed to be sleeping in a bouncer seat for more than 8 minutes and was not moved when it was brought to the caregivers attention.

During inspection three rooms did not have adequate lighting during naptime to see the children.

During inspection a 2m old infant and a 5m old infant were asleep in their pack n play covered with a blanket. The 2m old also had 2 empty bottles and 1 full bottle at the foot of the bed.

Individual storage was not available for children's personal belongings.
