The parent was not notified of an injury that required medical treatment by a health care professional. Management notified a relative during pick-up instead.

The standard was evaluated for a standard investigation and was found not compliant. Based on interviews with staff and a parent, it was determined that the caregiver did not demonstrate good judgment when a child was placed in the bathroom with the door closed as the caregiver stood in front of the bathroom door to speak to the child about their inappropriate behavior.
It was determined through an investigation that a child grabbed a child roughly during naptime after the child had gone to the wrong nap mat.
During this inspection, a caregiver allowed children between 18-24 months engaged in screen time. The deficiency was corrected during the inspection when the caregiver ended the screen time and removed the tablet and transition the children to another activity.
Two infants were observed asleep in restrictive devices; one was asleep in a high chair and another infant was asleep while placed in an infant seat. After it was brought to the caregivers attention that young infants that are non-mobile must nap in a crib, the caregivers removed the infants and placed each infant in a face up position in a crib.
The outdoor playgrounds have 4 holes that may cause a tripping hazard.
Two infant bottles were labeled. Labeling the children's bottle with the child's first and last name will reduce the risk of infants' bottle feed the incorrect formula and causing an allergic reaction.

Three infants did not have current infant feeding instructions. The feeding instructions were last shared with the parents March 2024. Caregivers reviewing the feeding instructions at least once a month ensure the caregivers have the most recent feeding instruction instructions feed the infants the proper nutrients each day at the operation.
This standard was tasked over for this investigation and found to be deficient. It was determined during the investigation that the caregiver in charge of the child did not properly communicate with the incoming staff or parent about the whereabouts of the child's device when needed. This caused confusion overall.
During the inspection time we observed three cribs without crib bedding present. The sheets had been washed and were placed back on the cribs during the inspection time.
At the time of inspection infants were observed not having their hands washed after diaper changes. The caregivers also did not wash their hands following the diapering procedures.
This standard was tasked over for this investigation and found to be deficient during the physical inspection. A few of the infants in care were observed crying for a period of time, placed in a rocker for over 15 minutes, and observed crying and standing in their cribs.
