This standard was evaluated as part of the investigation and was found deficient. Through review of video and interviews with staff, it was determined caregivers were not supervising a child in the fenced outdoor walkways of the operation; the child was alone for eight minutes.

This standard was evaluated for the investigation and was found to be deficient. Through review of video, and interviews with the parent of the child and staff, it was determined a caregiver grabbed and pulled a child resulting in a bruise to the child?s arm.

Through review of video of the incident and interviews with children, staff and parents, it was determined a caregiver used one hand to grab a child?s forearm and their other hand to grab the child?s knee. The caregiver then pulled the child up into the air, carried the child, and placed the child on their bottom on the rug.
