Six of the eight children identified with food allergies did not have food allergy emergency plans

This standard was evaluated with regards to an investigation and found to be deficient. A caregiver broke her artificial fingernail and a child was found with the nail piece in her mouth. The child was not injured and the operation discussed with the caregiver on how that could have better been handled.

This standard was evaluated with regards to an investigation and found to be deficient. A caregiver used her leg to keep a child on his map during nap time.

The operation interfered with the DFPS investigation when they would not allow the investigator to view the videotape of the entire day in which the alleged incident occurred.

During an DFPS investigation, it was determined that a caregiver used a cleaning product and then left it on a shelf. A child was observed playing with toys on that shelf.

While conducting an investigation, DFPS was not allowed to make a copy of the operation's videotape of the day in which the alleged incident occurred.

During an investigation conducted by the Department of Family and Protective Services, it was determined that the operation failed to make a report to the department after concerns were voiced regarding the treatment of children.
