Based on information obtained during a DFPS investigation it was determined that a caregiver was neglectful in providing the level of supervision required to prevent serious bodily harm.

The deficiency was corrected during the inspection when the caregiver and the director moved the children to a safe area in the classrooms to rest. Blocking exit and pathways may cause a hazard when children have to evacuate the classrooms during an emergency.

Children and Caregivers said hand washing using soap and running water was not complete prior to eating lunch.

A caregiver did not wash a young child's hands after a diaper change. The deficiency was corrected during the inspection when the director escorted the young child to the hand washing sink to wash the young child's hands with soap and running water. Washing hands after each diaper change helps reduce the risk of spreading germs.

Caregivers are not maintaining infant feeding instruction with the parent at least once every 30 days. Written communication from the parent maintained monthly by staff will help caregivers have a clear understand of when, where, and what to feed infants that are not ready for table food.

This standard was tasked over for this investigation and found to be deficient. After reviewing video footage during this investigation, it was observed that the child was supervised and re-directed as needed, but then was not observed properly and allowed to stand in the easel for a period of time, which assisted in the injury.

Two staff did not have a notarized affidavit for employees on file.

Two staff did not have documentation acknowledging orientation was complete.

A staff did not have a photo id on file.

Based on a review of information obtained during a DFPS investigation it was determined that the caregiver had requested assistance with the classroom management and the front office did not respond in a timely manner, thus not ensuring that the caregiver was able to appropriately handle the assignment to the age group in question.

Based on a review of the information obtained during a DFPS investigation it was determined that a caregiver used prohibited punishment while pulling a child as a form of redirection on multiple occasions.

At the time of inspection, the operation did not have an approval letter from the Fire Marshal allowing children to be cared for above ground available for review.

This standard was found to be deficient as the caregiver did not display appropriate supervision of the classroom after she spoke to the 2 children involved when they were behind the easel board. The caregiver walked away without ensuring that both children removed themselves from behind the easel board, which resulted in one child touching the private parts of another child while the children were in care.
