This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined a caregiver was not competent in their job duties when they gave a bottle of breast milk to the wrong infant.

The caregiver at the time of the bottle mix-up was not familiar with the child and did not know the name of the baby being fed. The center has a system in place with the infant caregivers to ensure that a name to face is conducted when bottle feeding. Call out the name on the bottle, identify the child, pick up the child, and match the bottle and child together.
