Based on the interviews with the staff, parent, child witness, the victim, and the video, the operation was found not to be in compliance with the standard evaluated. A young child found a small round object in the gym and placed the small round object in their nose. The young child received treatment from a health care professional to retrieve the small object out of the young child's nostril.

The director completed 15.5 hours of annual training.
Three of the four infants had current feeding schedules. This was corrected at inspection when the parent emailed a copy of the missing feeding schedule for the one infant who did not have a current feeding schedule.
This standard was tasked over for this investigation and found to be deficient. After review of the video footage from the date and time of the allegation, it was determined that the caregiver did somewhat hit or smack the child on his side leg before picking him up and taking over the sink. At the time of the investigation inspection compliance had already been met, as the caregiver was dismissed immediately following the incident the day before. A new caregiver is now present in the classroom and additional training has been provided to the caregivers.
The school's last fire inspection by a local fire marshal with the City of McKinney was on 6/23/21. No other fire inspection has been conducted at the center at this time.
