Through interviews with staff and children, it was determined that a caregiver took a child into a restroom with an obstructed view to discipline a child. It was determined the caregiver put their hands on the child's shoulders and pushed the child against the restroom wall which caused the child to hit the back of their head on the wall.

In the younger infant classrooom, the feeding instruction for the children present were not signed by a parent or had not been updated with the last 30 days.
In the older infant room, sheets in two cribs had food particulate on them despite none of the children in care having used the cribs today.

Broken toys and equipment were located on the playgrounds. These presented pinching hazards and due to being broken, cannot be used according to manufacturer's instructions. Several play structures had missing bolts in toys attached to play structures. Several uncovered holes were located in the ground that presented a tripping hazard. Animal feces were located on the splashpad accessible to children. The ground under the edge of the spash pad had eroded and presents rough surfaces that may scratch children and has a wire accessible to children.
In the older infant room, the fridge that stores children's food and drinks had uncovered food and was not clean.
The operation did not have a health inspection on file or proof that a health inspection is not required.
The operational policies were missing the Safe sleep policy for infants birth to 12 months old.
The operation did not have a current gas leak inspection on file or proof that a gas leak test is not required.
On the school age playground, there were 4 slides that exceeded 5 feet in height. The depth of the mulch under the slides was 5 to 7 inches in depth.
In an infant room, the bottles were not labelled with the infants' names.
On the toddler playground, there were protruding bolts coming out of the edging materials surrounding the playground. On the PreK playground, on the ground, there were electrical wires in a round box that did not have a cover on them.
The operational policies were missing the criteria that the operation will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play.
