This standard was evaluated as part of the investigation and was found to be deficient. Based on review of video, and interviews of children and staff, it was determined a caregiver left a child unsupervised in a classroom. The child left the building where their classroom was located. The child was found by another caregiver on the concrete path between the two buildings of the operation. The child was unsupervised for 6 minutes and 59 seconds.

Three of the eight rooms did not have emergency evacuation and relocation diagrams posted in the room. This was corrected at inspection when the operation printed out new diagrams and posted them in the rooms.

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff and children, and review of photos, it was determined that a caregiver had allowed children to sit on a fence to take photos on 3/4/2024. When a child was trying to sit on the same fence on 3/5/2024, the child fell off the fence and received two fractures in their wrist.

The use zones of five pieces of stationary equipment overlapped use zones of nearby stationary equipment. This was corrected at inspection when the operation put caution tape around the equipment to prevent children from using the equipment until the installer can come back out to the operation to properly install the equipment.

The phone app that the operation uses to track children when transporting children did not have medical transport and treatment authorization forms that could be pulled up on the app. This was corrected at inspection when the operation made paper copies of the medical authorizations to carry on the school vans.
The school van, manufactured in 2014, had a child safety alarm at the back on the vehicle but it was not operating.
The standard was evaluated as part of a standard investigation and was not found compliant. Based on the interviews with parents, staff, and a video, it has been determined that the caregiver did not use good judgement to redirect a child which resulted in the child sustaining an injury.
