During the inspection of a playground at the operation, the turf had holes in it in the active play area, presenting a tripping hazard to children.

This standard was evaluated during the investigation and was found to be deficient. Caregivers allowed children to pick up and drink from sippy cups which have been left out in the classroom by other children.

This standard was evaluated during the investigation and was found to be deficient. A child was bitten multiple times after caregivers did not properly supervise children in care.
Based on interviews with staff, the standard was found not to be in compliance. An assistant caregiver did not communicate with the lead caregiver or management that a child had fallen on the ground and hurt his/her shoulder during outdoor activities. The parents informed management the next day that the child received a fractured arm.
Based on the operation's video footage, a caregiver did not redirect a child in a positive manner. The caregiver was observed redirecting a child aggressively by grabbing and lifting the child by the wrist from the carpet to the trash can to throw away a tissue then escorted the child to a chair in a corner of the classroom to sit for timeout.
This standard was tasked over for this investigation and found to be deficient. A child was left unattended in his for approximately 14 minutes while his classmates were outside of the classroom. The child was found unattended in the class by his parent. The operation and director has implemented additional training and monitoring for the caregiver that was left iin charge. Observation of proper supervision was observed during the inspection time as well.
This standard was tasked over for this investigation and found to be deficient. The child involved in this incident was observed left in time out for more than the allowed number of minutes based on his age of 2 years; more than 2 minutes.
This standard was tasked over for this investigation and found to be deficient. After reviewing footage of the timeframe leading up to the incident, the caregiver in the classroom was observed sweeping and cleaning for a long period of time; not interacting with or supervising the children fully at this time.
An operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility. The person was removed from the classroom during the inspection. An administrative penalty in the amount of 100% of the daily maximum. per HRC 42.078(a-2) will be recommended as a result of the citation.
