Two infants who were not on table food yet had feeding instructions that were not updated monthly.

Out of eight caregivers' files reviewed, one did not have training in the following required topics: abuse and neglect, communicable diseases, food allergies, and handling hazardous materials. This was corrected during the inspection as the caregiver is currently on leave.

Out of eight children's records reviewed, one child was missing an admission date, one was missing an an updated immunization record, and one was missing a physician's health statement.

There was a bottle of cleaning solution in an unlocked cabinet where children could access it. This was corrected during the inspection when the caregiver moved the bottle to a drawer out of the reach of the children.

An electrical outlet in one of the infant rooms did not have a childproof cover. This was corrected during inspection when the director put a childproof cover on the outlet.

The infant daily reports did not have sections for each infant's mood and a summary of daily activities.

The parent handbook did not have the criteria for extreme weather that will prohibit children from going outside to play.

The children in the Toddler classroom were observed being changed on the floor where the children were observed playing. This was corrected when the caregiver sanitized the area and moved the diaper changing area.

A caregiver in care did not have current CPR/First Aid certification on file. The certification expired in May 2023.

Three caregivers observed with infants under 17 months old did not have proof of the required orientation and annual infant trainings.

During the inspection a 7 month old was observed sleeping on a sheet that was loose and could cause an entanglement. THis was corrected the caregivers pulling the sheet snugly.

During the inspection a 3 month old was observed in a swing sleep in a swing swaddled with a blanket. This was corrected by the blanket and the infant being removed.

During the inspection infants bottles were observed not labeled. This was corrected during the inspection by the caregiver labeling the bottles.

During the inspection two caregivers CPR certificates were evaluated and found to be taken online only.

A caregiver responsible for children during the inspection did not have documented proof of pre-service training or pre-service exemption.

During the inspection observed children in care enter the playground without the Designee conducting a daily safety check first. The daily grounds checklist was documented 2 out of 5 days. This was corrected during the inspection by the Designee checking the playground for hazards and documenting the check.

The last fire drill documented fire drill was completed in October 2021. This was corrected during the inspection by the operation completing a fire drill and exiting within the required time frame.

During the inspection the personnel files were not available for review.

During the inspection a recall rock n play was observed in the infant classroom. This was corrected during the inspection by the recall item being removed from the classroom and placed in the office

During the inspection one caregiver was on the playground in the care of eighteen ages 20 months to 3 years old. This was corrected by a second caregiver returning to the playground.

During the inspection caregivers files were not available during the inspection

During the inspection four children files were evaluated and did not have health statements.

During the inspection children were observed laying on mats that were not waterproof. This was corrected during the inspection by children being provided waterproof mats.

During the inspection infant bottles were not labeled.

During the inspection the operation was in care of five children over the capacity. This was corrected by the five children being picked up by parents.

During the inspection the infant feeding schedules were not signed and dated by the parent

During the inspection a caregiver was in care of children without a cleared background check. This was corrected by the caregiver leaving the operation.

During the inspection the infant classroom did not contain a rocker or adult size chair.

During the inspection one caregiver was in care of six ages 10 months to 24 months. This was corrected by two children being picked up by parents.

During the inspection the under sink counter in the infant room was covered with a piece of cloth which allowed infants accessible to storage items.

During the inspection it was found that playground area accessible to children in care contained a wooden broken chest, a head entrapment in the fence, and easy access to the alley behind the operation. The wooden chest was removed during the inspection.

During the inspection the activity plans for each age group were not posted in the classrooms
