Infants were observed sitting in seats at a large highchair table for at least 1.5 hours. The care giver was observed on her phone and not interacting with the infants.

Toddler classroom 12 to 24 months does not have outside time as part of their daily activity plan.

I observed a food allergy plan that was not signed and dated by parent of child.

I observed 4 televisions in 4 different classrooms that are not anchored to avoid them from tipping over.

Infant caregivers have not been documenting infant daily care sheets to share with parents at the end of the day.

An infant was asleep face down in the crib with a bib around their neck. The bib was flipped over the infant's head. The infant is able to roll on their own. This was corrected at inspection when the caregiver removed the bib.

Children under two years of age were watching TV at drop off time.

Two classrooms at drop off time were out of ratio, 28 children to one caregiver. mixed ages ranging from infants through pre-k children (3,4, 5 years) were out of ratio by 13 children. Specified age group was three years.

Two caregivers left the classroom of children napping unsupervised for two minutes.

An infant classroom of nine 11-17m old infants and a toddler classroom of eleven 18-23m old children were out of ratio when a new employee without preservice training or CPR/Frist Aid certification was used to meet ratios. The maximum number children for 11-17 m children one qualified caregiver may supervise is five. The maximum number of toddler 18m-23m children one qualified caregiver may supervise is nine.

This standard was evaluated and found to be out of compliance. It was found that a caregiver was dancing on the table and listening to inappropriate music while in charge of a classroom.

A child was injured while in care at the operation and required medical attention. The operation failed to report the incident to Child Care Regulation.
