Two observations of two different employes had same date of 8/29/23 and same time frame T1B 8:18 am - 8:50 am and T2B 8:20 - 8:50 AM time. One observation did not have a caregiver name. Some of the observations have the name of a director assistant and not the director's name, conducting the observations. There have not been any monthly conferences conducted with observed caregivers.
