Children were on a field trip and did not have anything on them identifying them to the daycare.
Employees were not appropriately supervising children when they were seen on their personal phones during a field trip.
Staff did not demonstrate good judgement when a child climbed up on chairs that were stacked. Staff saw the child and went to her but did not pick up the child to remove her off the chairs. Staff stood in front of the chairs until when the child fell from the chairs, causing her to hurt herself. When asked why she did not pick up the child, she stated that she does not like to lay hands on the children and will give them their space.
