An employee did not show good judgment when she picked up a child by the arm.

Children were on a field trip and did not have anything on them identifying them to the daycare.

Employees were not appropriately supervising children when they were seen on their personal phones during a field trip.

Staff did not demonstrate good judgement when a child climbed up on chairs that were stacked. Staff saw the child and went to her but did not pick up the child to remove her off the chairs. Staff stood in front of the chairs until when the child fell from the chairs, causing her to hurt herself. When asked why she did not pick up the child, she stated that she does not like to lay hands on the children and will give them their space.

Upon reviewing children's records, 2 out of 10 children's records did not have a healthcare statement available for review. This standard was recited.

Upon reviewing children's records, 10 out of 10 children's records did not include an address for the emergency contact.

Upon reviewing children's records, 3 out of 10 children's records did not have an immunization record available for review. This standard was recited.

During my inspection, I observed at least 2 bottles of cleaning sanitizer accessible to children.

During naptime, I observed one caregiver with 11 children between the ages of 12 through 18 months of age. This was corrected at inspection.

A 3 year old child was left unsupervised in a classroom.

Operation failed to make a report of a communicable illness within the required 2 day timeframe.

Two toddlers were unsupervised in a classroom. Both children were by themselves with no staff.

An infant under 12 months of age is being allowed to sleep while using a small pillow under his head.

There was no documentation for current fire drills. The last fire drill was documented in August.

Director credential was not renewed timely.

Director credential expired on 10/31/21. A new credential has not been provided.

A 2 year old child was left unsupervised in a room by himself.
