A caregiver was caring for 8 children of varying ages in the infant room during naptime. The ages of the children were two infants between 12-17 months of age, two toddler children, three pre-k children, and one school aged child. According to the ratio chart when 12 or fewer children are in care two caregivers are required to supervise the group of children due to the ages of the children. Note: A second caregiver stepped into the infant room during the inspection.

Six staff files were reviewed and the following information was not available for review at the time of inspection: -Transportation training for Director -16 to 24 hour of pre-service training for 3 caregivers - 1 to 4 hours of annual trainings for 2 caregivers including trainings of safe sleep and abuse/neglect
Six children's files were reviewed. All files are missing the updated water activities information including the children's ability to swim details and special care information. Three out of six files reviewed are missing updated immunization records.
Based on the information obtained during DFPS investigation it was determined a child was left unattended in a vehicle.

Five staff files were reviewed: one staff members is missing 10 hours of annual training and a second staff member is missing 8 hours of annual trainings including trainings on the following topics: Management Hazardous materials Food allergies Administering medications Emergency Preparedness Building and Safety Premises Safety Communicable Diseases
Five children's records were reviewed. One files is missing emergency contact name, address and telephone number. A second file is missing immunization records and emergency food allergy plan and/or statement child no longer has a food allergy.
The role of a volunteer changed to an employee role two months ago. Operation has not submitted a renewal background check with the employee's current role.
Last Fire Inspection was conducted on 12/7/2020.

An infant was observed asleep in a bouncer and was covered with a blanket.
During nap time in the after school group, the mats were arranged right next to each other with no sufficient walk and work space for caregivers between each mat.

Emergency Preparedness Plan is missing the following sections: 2) Communication; (3) How your staff will evacuate and relocate with the essential documentation (4) How your staff will continue to care for the children until each child has been released; and (5) How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.
Ratios were not being met when an infant was mixed in a group in which the oldest child was 4 years old.
