The Children's Products Certification form was not completed to prove that the director has reviewed each of the recall notices issued by the CPSC and that there are no unsafe products in the home.

The operational policies reviewed did not include information about the presence of a dog inside the home.

There was no statement of health from a local veterinarian for a small dog present at the home.

There were no records available for review for three children in care.

Fire drill practices were being conducted every three months instead of monthly.

The mats where children were laying on were not labeled with the child's name or numbered. Note: This was CORRECTED during the inspection as the mats were labeled with a number and a number/child assignment map was created.

The director was caring for a total of 12 children. In the group 10 of those children were between the ages of 18 months - 3 years of age. According to the ratio chart the maximum number of children one caregiver may supervise between ages 18 momths-3 years is 8.

The Children's products certification Form was not current. The date on the form provided was 2018.

There was no file available for review for a child that was in care. Children's records reviewed were missing the following information: Two files were missing the Authorization to obtain emergency medical care and to transport the child for emergency medical treatment. Two files were missing the child's health statement. One file was missing the child's immunization record.

The director was missing 30 clock hours of annual training for the year of 2/24/2020-2/23/2021.

The director did not have current training in Pediatric first aid with rescue breathing and choking; and Pediatric cardiopulmonary resuscitation (CPR). The expiration date on the certificate of training reviewed was January 2020.
