Caregiver was observed to be sitting in a chair and not intervening while children where observed to be running in the classroom and throwing different toys in the classroom at each other.

An infant under 12 months of age was asleep in a crib with a pacifier clip attached to them while asleep.

Children were observed to not have water made available to them during their outside time.

Based on the information obtained during the investigation it was determined that a caregiver did not intervene when two children were throwing items at each other that caused both children to have an injury.

There is sufficient evidence to support that the operation did not share a copy of the report to the child's parent to obtain signature on the report indicating the parent had reviewed and received the report within 48 hours of the incident. The incident occurred on June 2nd, and the parents did not review or received the report until June 16th

A child with a known food allergy diagnosed by a health care professional was not observed to be posted in the child's classroom with a known food allergy.

Three toilets were observed to be broken in three different classrooms.

Two infants were observed to not have current feeding instructions.

Three children's records did not have parent's rights forms in their file.

Two bottles were observed to not be labeled with the infant's names.

One child's file was observed to not have an address for the emergency contact.

Immunization was not observed in one child's file that was evaluated.

Caregiver was observed to not wash their hands after cleaning a child's nose that was bleeding.

Caregiver was observed to not wash their hands after diaper change.

The unitary surfacing was observed to be in need of repair and was not installed according to manufactures specifications.

The school age classroom did not have a list with the children's names and ages.

4 out of 5 employee files reviewed did not have a completed affidavit Form 2912 in their file.

Six children's records were reviewed. Three children have immunization records that are outdated based on current age.

The loose-fill material surrounding the composite play structure is below the required depth of nine inches.

The loose-fill material surrounding the swing set area is below the required depth of six inches.

The operation did not have a fire inspection conducted prior to issuance of the initial permit.
