The household member lacked documentation of completion of orientation.

A sampling of children records were evaluated. Two children's immunization records were observed incomplete.Discussed the names of the children and their missing immunizations with the primary caregiver.

The operation did not have a written activity plan.

The primary caregiver lacked 19 hours of training to complete last years annual training.

The operational policies were evaluated and lacked the following written procedures: 1) Safe sleep policy; 2) Promotion of indoor and outdoor physical activity (A-C); 3) procedures for parents to visit the home any time during hours of operation to observe their child, program activities, the home, the premises, and equipment without having to secure prior approval; and 4) the HHSC website (www.hhs.texas.gov).

This standard was reviewed on 3/17/2022 as a follow-up to a previous inspection and found deficient. The metal frame of the swing set that was taken down is rusted and on the playground accessible to children. It needs to be removed from the playground. A three person wooden swing with stand was observed on the playground with wooden boards missing from the seat and the back of the swing posing an entrapment risk to children. This was corrected at inspection as the swing was taken off the frame and removed from the playground.

A metal cover on a vent attached to the outside of the home is not secured and is sticking out. One of the legs on the frame of the swing set is bent making the swing set unstable. This swing set needs to be removed from the playground. The roof tops of two play houses have large areas of missing plasttic as well as cracked plastic. One piece of equipment with a slide has a hole in the plastic at the end of the slide as well as another hole toward the top of the piece of equipment. The a/c unit on the playground does not have a safeguard around the unit to prevent the children from having access. The gates to the fence around the playground are not secured to prevent the children from exiting the playground.

The dog in the home did not have documentation on file of current vaccinations.

The Emergency Preparedness Plan lacked a written procedure for how you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

The operation did not have documentation on file of a statement of health from a local veterinarian for the dog in the home.

The last observed annual certification that you reviewed the CPSC Recall list was 2014. You must certify annually that you have reviewed the CPSC recall list and their are no recalled items in your home.

The substitute lacked documentation of completion of CPR training. This was corrected at inspection as this substitutes last date of employment was given as 5/25/2021.
