Caregiver did not use proper EBI method, child in care was pulled inappropriately this was confirmed by other residents in care.

Four children in care indicated a staff member uses profane language with the children in care. A second staff member indicated profane language just slips out sometimes. The staff member who allegedly uses profane language claims to have not done so since being investigated by Licensing (Spring 2021), however three of the four children in care claiming profane language usage were admitted to the operation after Spring 2021 and the staff who admitted there's profane language her was hired in Spring 2022.

Staff should not use EBI for getting a child to comply with them.

Child confirmed staff held child around the neck.

A staff member pushed and yelled at a child in care.

The normalcy training and trauma informed care training found in the employee file were over two months late.

One staff did not receive their emergency behavior training within the 180-day timeframe.

It was found that EBI training was incomplete for 2 staff post-6 months from the last training.

Multiple children in care indicated care givers at the operation use profane language with the children in care and had details about what words were said and it how it made the children in care feel. Multiple staff admitted to using profane language in the presence of children in care.
