During the unannounced inspection, child and staff were not washing hands after diaper changing. This corrected during the inspection.

All children's nap mats/bedding were observed on a shelf touching one another. This was corrected at inspection when a caregiver placed each nap mat in a separate plastic trash bag and then labled the trash bag with the child's name.
It was observed that playground safety checks had not been being documented monthly as required.
Upon review of employee files, it was discovered that 3 employees were lacking annual trainings hours in required topics.
It was observed that the children in the program had not been signed in or out for a period of at least 3 days. This was corrected at inspection when the caregiver signed all of the children in for the day.
Both caregivers in the toddler room were unaware how many children were present under their care. There were 16 children ranging from 18 months to 3 years old. The specified age group was 2 years old.
